Pharmacist
Job Brief:
We are looking for a knowledgeable and compassionate Pharmacist to join our team. As a Pharmacist, you will be responsible for dispensing medications, providing medication therapy management, and counseling patients on proper medication use. You will play a vital role in ensuring the safe and effective use of medications and improving patient outcomes.
Responsibilities:
- Dispense prescription medications accurately and efficiently.
- Review medication orders for accuracy, appropriateness, and interactions.
- Provide counseling and education to patients on medication use, dosage, side effects, and precautions.
- Collaborate with healthcare providers to optimize medication therapy and patient outcomes.
- Conduct medication therapy management services, including medication reviews and adherence assessments.
- Monitor patient health and medication regimens to identify and prevent medication-related problems.
- Maintain accurate patient records and medication profiles.
- Manage pharmacy inventory and order medications and supplies as needed.
- Ensure compliance with federal, state, and local regulations governing pharmacy practice.
- Participate in immunization and wellness programs.
- Provide drug information and clinical support to healthcare professionals.
- Perform medication reconciliation and discharge counseling for hospitalized patients.
- Assist with medication therapy management for chronic disease states.
- Collaborate with interdisciplinary healthcare teams to provide comprehensive patient care.
- Stay updated on new medications, therapies, and treatment guidelines.
Qualifications and Requirements:
- Doctor of Pharmacy (PharmD) degree from an accredited pharmacy program.
- Licensed pharmacist in good standing with the state board of pharmacy.
- Strong knowledge of pharmacology, pharmacotherapy, and medication management.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with other healthcare professionals.
- Attention to detail and accuracy.
- Ability to prioritize and manage multiple tasks.
